Jacob Lewis passed away on April 13th in Phoenix, AZ. with his parents at his side. Jake was born August 28,1989 to Dennis and Terry Lewis in Bloomington, IL.

Jake has two sisters, Ashleigh (Brandon) Huber of Holland, OH and Rachel (Tim) Webb of Simpsonville, SC.

He has three nephews, Owen Huber, Luke Webb and Caleb Webb that he adored.

Jake was an avid sports fan. He loved his Chicago Bulls and Cubs and the Green Bay Packers.

Jake graduated from Knox College in Galesburg, IL with an Economics degree. He participated on the basketball and golf teams during his college career.

Jake loved to spend time with his family on vacations to Lake of the Ozarks and their annual Christmas week together.

Jake moved to Arizona in 2013 to work for State Farm in Auto Underwriting.

He is survived by his paternal grandmother, Marlene Scott of Prophetstown, IL. His maternal grandparents, Larry and JoAnn Frederick of Prophetstown, numerous aunts, uncles, cousins and friends.

Jake lost a battle with cancer. During his illness he fought the cancer with dignity and grace.

Donations can be made to the American Cancer Society, American Brain Tumors Association or Barrow Brain Institute.

  1. Lewis Family so sorry to hear of Jacob’s passing. I always think of him as the young sportsmate with my grandson Zach. May God comfort you at this time and the days and years ahead.
